Numbing Cream Order Form
If you’ve booked in for a cosmetic tattooing service, your numbing cream will be specially compounded and sent directly to our salon before your appointment.
To have this prepared, you’ll need to fill out a medical form which will be securely sent to both our salon and the compounding pharmacy. Once submitted, the pharmacy may contact you directly if they require further information.
Please allow at least 7 days between submitting this form and your appointment date.
This allows enough time for your form to be assessed by the pharmacy, for the cream to be compounded, and for it to arrive at the salon in time for your appointment. If your appointment is in less than 7 days, you must still complete this form, but please also contact the salon directly so we can advise on next steps.
How it works:
1. Fill in the short form below.
2. Once submitted, your details will be securely shared with Synergy Compounding Pharmacy.
3. The pharmacy will compound your numbing cream and post it to our salon, ready for your appointment.
All information remains confidential and is only used to assess suitability for the numbing cream.
